Meanwhile, Se-jin grows more affectionate toward the woman he believes is Mi-ji, and Ho-su begins to suspect something is off.

Episode 5 begins with a tender yet haunting flashback—Ho-su, still a young boy, finds himself in a life-altering accident alongside his father. Years later, even after multiple surgeries, the doctors advise him and his mother that further treatment could worsen his condition. He’s told to avoid hiking or running for his own safety. Despite this, as his classmates plan a field trip to a mountain, Ho-su impulsively decides to join the hike, determined to reach the peak. Mi-rae, concerned, asks why he would risk it, but Ho-su remains firm—he’s going.
On the trail, Mi-ji, who’s secretly watching over him, tries to strike up a conversation, but a fatigued and frustrated Ho-su snaps at her. Her heart sinks. Cut to the present: on the rooftop with “Mi-rae” (really Mi-ji), Ho-su opens up about his first love—Mi-ji. When she presses for details, he starts to explain, but is interrupted by a call from the hospital. Ro-sa needs minor surgery for gallstones, prompting memories of her childhood hospitalisation, the details of which remain unknown.


Mi-ji, still pretending to be Mi-rae, gently probes Ho-su for more about the hike. He says he fell for Mi-ji that day—her strength and quiet encouragement meant everything to him. Back at work, Mi-ji’s stressful double life intensifies. The company CEO demands updates on a major project, and her supervisor, Gyeong-min, piles on pressure, clearly setting her up to fail. Mi-ji panics, barely holding it together.
Meanwhile, Mi-rae is living a quieter life in the countryside, making jams and working on farms. But word is spreading that she and Se-jin are getting close. Gyeong-gu notices her growing attachment to Se-jin and becomes visibly disturbed. Soon, rumours swirl that Se-jin and Mi-ji (really Mi-rae) are dating.


That night at the hospital, Mi-rae hands her phone to her grandmother, who wants to speak with Mi-ji. When Mi-ji vents about how overwhelmed she feels, her grandmother gently reminds her of a promise. Mi-rae overhears and is puzzled. When she asks about it, the grandmother smiles and says it’s a secret.
The next day, Mi-ji finds some helpful documents on her desk and realises her colleague Tae-i quietly stepped in to assist. Desperate, she confides in him, even admitting she’s been struggling with depression. Tae-i, touched by the vulnerability of the woman he believes is Mi-rae—the same one who once supported him—agrees to help her complete the report.


As Mi-ji works late, Ho-su wrestles with confusion—does he like Mi-rae, or is it Mi-ji’s soul he’s drawn to, hiding in Mi-rae’s skin? Seeking clarity, he visits Mi-rae in the countryside, still thinking she’s Mi-ji. He tells her how Mi-ji’s encouragement during that mountain climb pushed him to finish the hike despite the pain. Mi-rae, hearing this, is flooded with guilt for hiding the truth from him.
Later that night, Mi-ji calls Mi-rae, pressing her to explain what happened at the peak. Mi-rae reveals Ho-su had spoken about how Mi-ji changed his life. Mi-ji is stunned—she had no idea he had feelings for her, especially since she assumed Mi-rae and Ho-su once dated. But Mi-rae clarifies: they never did. Ho-su’s heart has always belonged to Mi-ji.


When Mi-ji brings up a moment during the school race where Mi-rae hugged Ho-su, Mi-rae confesses she had collapsed from illness just before the CSAT and never even finished the race. That moment, like so many others, had been misunderstood. The sisters realize how much pain could’ve been spared if only they had spoken to each other honestly.
Shaken, Mi-ji rushes to Ho-su’s place but injures her ankle on the way—remnants of her past surgery flaring up again. Meanwhile, back at work, Tae-gwan sees an opportunity to advance his career and seizes control of the HQ project after the CEO dangles a promotion. But Ho-su, now Ro-sa’s lawyer, starts pressuring him to revise the proposal thoroughly. Tae-gwan, threatened, turns to Lee Chung-gu, hinting at brewing corporate sabotage.


Back in town, Mi-rae hears the dating rumours from her own mother, who seems to believe them. Overwhelmed, Mi-rae recalls the years of judgment and gossip she endured in Seoul and finally snaps. The next day, she walks into her workplace and quits on the spot, leaving Se-jin stunned and concerned.
In Seoul, Mi-ji gets criticised by Gyeong-min for the report, even though it’s clear he’s trying to discredit her and steal her work. That evening, she finds herself drinking soju at a bar, lost in thought. Tae-i finds her there, and she finally opens up. She talks about the years she spent locked away in her room, the guilt of missing her grandmother’s stroke, and the promise she made—to never run away again. Tae-i, moved, silently promises to support her however he can.


That same night, Ji-yun meets Ho-su and brings up Mi-rae. Ho-su makes it clear—they weren’t close even in school. Ji-yun takes the opportunity to hint at her own interest in him, but Ho-su doesn’t respond. Instead, he gets a call from “Mi-rae” (actually Mi-ji), and noticing her drunken state, rushes to meet her at his apartment.
There, Mi-ji finally asks about what really happened during the mountain hike. Ho-su shares that when everyone treated him like a fragile boy, it was Mi-ji who waited at the summit, not rushing him, not pitying him, just believing in him. That moment, he says, made him feel normal. Mi-ji, tearing up, finally confesses—he was the reason she stepped out of her own darkness. Then she says it: “I liked you.” Ho-su stands stunned, hearing a long-awaited confession from the lips of the woman he thinks is Mi—rae—unaware he’s hearing the truth from Mi-ji, the girl who has always held his heart.

Through Ho-su’s heartfelt recollection of the mountain hike and the quiet resilience he found in Mi-ji’s presence, we finally see the roots of his affection and confusion unravel. The flashbacks serve as emotional anchors, blending the past and present with a raw tenderness. Meanwhile, Mi-ji and Mi-rae’s ongoing identity swap continues to fuel misunderstandings and heartbreak—not just in their romantic lives, but also in their careers and relationships with those around them.


What stands out in this episode is its intimate portrayal of emotional vulnerability. Mi-ji’s panic at work, Mi-rae’s breakdown after enduring gossip, and Ho-su’s quiet torment all reflect how internal battles often remain hidden until they explode. Tae-i’s empathy and support for Mi-ji hint at a possible emotional shift, while Ji-yun’s subtle move toward Ho-su adds a new layer to the romantic tension. The episode builds momentum steadily and ends on a poignant note, Mi-ji’s confession to a love that has lingered for years, spoken at last but still misheard.
